Paul C. Bastante is a Certified Aging-in-Place Specialist (CAPS) and educator with a passion for connecting healthcare professionals to practical, real-world solutions that improve patient outcomes. His experience bridging clinical care and home accessibility inspired the creation of The AgeWise Institute — a comprehensive educational platform designed to elevate how we prepare patients for life after discharge.
